Student forms

The forms must be completed following this procedure:

  1. Download the relevant file
  2. Fill out the form digitally using the interactive fields
  3. Validate the document with one of the following methods:
    • Digital signature
    • Handwritten signature (scan the document and save it in PDF format)
  4. Send the PDF document via email to whom it concerns

It is advised to fill out the forms using the free PDF reader "Acrobat Reader".

N.B. Students enrolled in the “Computer Science (L-31) Degree Course” must use the forms published on the site at the following link.
